We are seeking a compassionate, caring, and dedicated Behavioral Health Care Manager to join our collaborative, multidisciplinary Tailored Care Management (TCM) team in Lenoir and Duplin counties.
What You’ll Do:
PORT Health’s Tailored Care Management (TCM) team provides whole-person, integrated health care through evaluation, assessment, care planning, coordination, tracking, and monitoring outcomes of individuals served. The Care Manager focuses on closing gaps for physical and mental health, social determinants of health, and other non-health-related needs. This position travels within Lenoir and Duplin counties.
How You’ll Benefit:
You’ll earn a competitive salary between $51,000 and $53,000, depending on skills and experience . This full-time position offers a flexible schedule Monday-Friday from 8 am-5 pm. We also offer competitive benefits for eligible positions.
